REQUEST FOR PROPOSAL

INVITATION TO BID for Phase I - Sanitary Sewer Improvements for the Village of Sunrise Beach, Missouri Sealed proposals, addressed to the Village of Sunrise Beach, will be received until 1:00pm, Thursday May 9, 2013, at City Hall, 16537 N. Hwy. 5, Sunrise Beach, Missouri, after which they will be publicly opened. The contract to construct the sanitary system improvements will include one contract. The shall include the supply and installation of a 50,000 gallon per day extended aeration pre-manufactured wastewater treatment plant and all site improvements, and the construction of a pressure wastewater collection system that includes individual grinder pump stations, 1.5”, 2”, 3”, and 4” sewer lines, and all appertanences to make a complete sewer collection and treatment system.

The project is located in Camden County. The project must be completed in 270 days from the time of Notice to Proceed. The Engineer shall conduct a pre-bid conference at City Hall, 16537 N. Hwy. 5, Sunrise Beach, Missouri, at 10:00am, Monday April 29, 2013. All Bidders are encouraged to attend if they have questions or desire to comment on the project.

Copies of the Contract Documents and Specifications required for bidding purposes may be obtained from the office of Schultz Surveying & Engineering, at 3287 Bagnell Dam Blvd. Suite A, Lake Ozark, MO 65049, upon payment of $175 for each set. All planholders upon returning such set in good condition within 10 days after the bid date shall be refunded $30.00. CDʼs with PDF Plans and Specs are available for a non-refundable cost of $75.00. All plans and specs, and CDʼs, shall be shipped priority mail. If bidder desires faster delivery, he may provide Fed Ex/UPS account information to engineer for desired delivery date. Payment will be made in cash in accordance with the provision of the terms of payment in the Contract. Proposals shall be submitted on the blank form in the Contract Documents provided for that purpose. Bids shall be accompanied by Bidderʼs certified check or Bid Bond in the amount of five percent (5%) of the Bid Price. The successful Bidder will be required to furnish Performance and Payment Bonds, each in the amount of one hundred percent (100%) of the Contract amount.

The successful Bidder will be required to furnish workerʼs compensation insurance per the minimum requirements of the State of Missouri, as well as general liability and automobile insurance in a minimum amount per the General Conditions of the Contract for Construction. The Owner shall have the right to waive informalities or irregularities in a Bid received and to a accept the Bid which, in the Ownerʼs judgement, is in the Ownerʼs own best interests. Nondiscrimination in Employment: Bidders on this work will be required to comply with the Presidentʼs Executive Order No. 11246. Requirements for bidders and contractors under this order are explained in the specifications.

This project was financed through the Missouri State Revolving Fund, established by the sale of Missouri Water Pollution Control bonds and federal Capitalization Grants to Missouri. Disadvantaged Business Enterprise - All bidders are required to comply with the “Six Good Faith Efforts”, as outlined in Division A, and submit the Disadvantaged Business Enterprise forms as part of their proposals in accordance with the Missouri State Revolving Fund and State Grant & Loan Program ʼs Procedures for Implementation of Disadvantaged Business Enterprise requirements. Anti-Lobby Act - “Sub recipients who request or receive from the grant recipient a subgrant, contract, or subcontract exceeding $100,000, at any tier under a federal grant shall comply with the Anti-Lobbying Act, Section 319 of Public Law 101-121, and file an Anti- Lobbying Certification form, and the Disclosure of Lobbying Activities form, if required, to the next tier above.”

Certificate must be included and submitted with the bid proposal. Domestic Products Procurement Law - All manufactured goods or commodities used or supplied in the performance of any contract or subcontract awarded on a project shall be manufactured, assembled or produced in the United States, unless obtaining Americanmade products would increase the cost of the contract by more than ten percent (10%). By signing of your bid you agree to the Domestic Products Procurement Law - RSMo. Certificate must be included and submitted with the bid proposal.
